Skilled Nursing
Our Registered Nurses and Licensed Vocational Nurses, combine the science of nursing
with the art of caring. They are qualified and highly trained to perform procedures
and interventions such as but not limited to:
- Wound care
- Injections
- Enteral Feeding
- Catheter Care
- Ostomy Care
- Post surgical Care
- Diabetic Care and Management
- Medication Management
- Care for chronic medical conditions
Certified Home Health Aide
Our CHHA’s are available to assist with patient’s personal care and hygiene needs
and light housekeeping under the direct supervision of a Registered Nurse or Therapist.
Physical Therapy
Our Physical Therapists will provide physician-prescribed treatment
aimed at development and restoration of muscle and joint function to improve mobility
and function. These include but are not limited to: gait and transfer training,
establishment of home exercise program, muscle strengthening, safety evaluation
and instructions on use of assistive device, and pain management.
Occupational Therapy
Our Occupational
Therapists will help patients regain the skills necessary for self care and activities
of daily living. Their focus is with patients who have upper extremity weakness,
limited range of motion, and decreased fine motor movements.
Speech Therapy
Our Speech Therapists
will assist patients with impaired communication skills. They also assist patients
with cognitive deficits, memory disturbances as well as patients with swallowing
difficulties and poor gag reflex.
Social Services
Our licensed Social Workers will assist patients with their social, emotional, financial
and environmental needs. Not only will they assist with counseling but they can
also provide referrals to community resources and support such as meals, transportation,
and homemaker assistance.
Other services such as Durable Medical Equipment, Dietician and Respiratory Services
may be arranged.
